Periapical testing helps identify the tooth and determine how far the infection has progressed. The test may involve:

• Percussion test: The dentist gently taps the tooth with a dental instrument to evaluate if there is any pain or sensitivity, which could indicate inflammation or infection. The test is gentle, ensuring you won’t experience discomfort.
• Palpation test: The dentist presses on the gums near the tooth’s root to check for tenderness, swelling, or signs of infection.
• Periapical radiographs (X-rays): This imaging technique captures detailed pictures of the tooth’s root and the surrounding bone, providing insight into infections, bone loss, or abscesses that may not be visible to the naked eye.

Here are cases when we prefer periapical testing over pulp vitality test.

Pulp testing focuses on whether the pulp is alive (vital), but periapical testing can reveal conditions like abscesses or cysts around the root.
• Failed root canal: If you continue to experience pain or symptoms after a root canal treatment, periapical testing helps identify whether there is a lingering infection or other issues around the root apex that may require retreatment or surgery. If so, you may need an endodontic retreatment to give your infected tooth a second chance.
• Suspected irreversible pulpitis: If we suspect a non-vital or dead pulp, we may recommend periapical and pulp testing to check for complications extending beyond the pulp.
• Post-trauma evaluation: In cases of dental trauma, especially involving the root, periapical testing evaluates the extent of damage to the surrounding tissues and bone. Pulp testing may not be enough if the root or bone around the root is affected.
• Endodontic treatment monitoring: Dr. Patel can rely on periapical tests to monitor healing after root canal therapy. Periapical testing assesses whether the tissues around the root are healing properly or if persistent infections or complications remain.

After periapical testing, the outcomes may indicate the need for a root canal retreatment. Sometimes, an apicoectomy is needed to remove the infected root tip and the surrounding tissues.
